- Adult Protective Services (APS) is a program designed to assist adults, 18 years and above, who are at risk.
- Risk factors include:
- Decreased physical health and mobility
- Isolation
- Dependency/caregiver stress
- Requirements for eligibility are:
- Must have a physical or mental disability
- Be at risk of harm or have an unmet need
- AND have no one willing and able to assist them responsibility
- Risk factors include:
- Residential Placements for Adults
- Article 81 Guardianship gives authority for a legal proceeding to proceed; based upon the concept of placement into the least restrictive environment
- When necessary, a guardian is appointed to meet the financial and/or physical needs on an incapacitated person.

- Personal Care Aides (PCA)/Consumer Directed Personal Care Program (CDPAP)
- PCA: a licensed physician submits an order for a PCA/CDPAP to the unit.
- A joint visit is conducted with a caseworker and Public Health nurse to assess
- Requirements for eligibility are:
- Must have straight Medicaid and one of the following:
- Waiver Program
- Level One Care
- Housekeeping and shopping
- OR be under the age of 21
- Must have straight Medicaid and one of the following:
- PCA: a licensed physician submits an order for a PCA/CDPAP to the unit.
